Chronic Disease Management

At Rural Health Group, we believe in bringing healthcare to the people who need it most – when and where they need it. That’s why we’ve partnered with FNQ PHN to deliver a new Chronic Disease Management (CDM) Program across Ravenshoe, Mount Garnet, Herberton, and surrounding communities.

Designed to support people living with chronic health conditions, this initiative shifts the focus from emergency care to proactive, preventative healthcare. Our goal is to empower patients and help them take control of their health through consistent, community-focused care.

What is Chronic Disease Management?

Chronic diseases such as diabetes, heart disease, and respiratory conditions require ongoing care. Our CDM Program supports patients through personalised health planning, regular check-ups, and coordinated access to Allied Health professionals like dietitians, diabetes educators, and more.

Accessible

We reach out to the community. Our health workers phone patients and travel locally to ensure everyone knows about the services available and gets support to attend appointments.

Patient-Centred

It all starts with an Annual Health Check. From there, patients work with their GP to develop a tailored Care Plan, which includes ongoing support and referrals.

Consistent

Care Plans are reviewed every 3–6 months and renewed annually to adapt to evolving health needs.

Why This Matters

Chronic conditions are some of the most pressing health issues in rural Australia. Many of our communities face barriers to accessing care until it’s urgent. The CDM Program changes that by:

  • Encouraging early detection and management
  • Reducing preventable hospitalisations
  • Improving quality of life for people living with chronic illness
